About The Position

The Pharmacy Division is dedicated to ensuring safe, effective, and cost-conscious medication use while collaborating with healthcare professionals to achieve optimal drug therapy outcomes. The Ideal candidate is a licensed pharmacist with a Pharm.D. or Bachelor’s degree in Pharmacy from an accredited college. They demonstrate strong clinical knowledge and a commitment to patient-centered care in an ambulatory oncology setting. While hospital pharmacy experience (6 months or more) is preferred, the candidate should excel in accuracy, communication, and collaboration within a multidisciplinary team. • RPH - Registered Pharmacist Texas State Board of Pharmacy. within 90 Days
• BLS - Basic Life Support American Heart Association (AHA). within 30 Days
• ACLS - Adv Cardiac Life Support American Heart Association (AHA). If working in one of the following departments: Pharmacy Inpatient (710348), Pharmacy OP ACB (710355), Pharmacy-West Houston (710657), Pharmacy-The Woodlands (710480), Pharmacy-NWH Sur&SP Care (711204). within 90 Days

Responsibilities

• Demonstrate strong interpersonal and customer service skills by treating patients with compassion, dignity, and respect.
• Maintain professional and appropriate communication, including language, tone, and volume, when interacting with patients, family members, healthcare providers, and other stakeholders.
• Communicate promptly with providers regarding prescription changes, patient concerns, medication-related issues, or other clinical needs as appropriate.
• Conduct comprehensive patient assessments, including: Review and maintenance of an updated home medication list. Assessment of medication adherence. Evaluation of patient quality of life and therapy outcomes.
• Assess the appropriateness of specialty medications and monitor patients throughout therapy to support achievement of desired clinical outcomes.
• Provide specialty medication counseling and education, including: Medication administration. Expected therapeutic outcomes. Potential adverse effects and their management.
• Participate in specialty clinical consultations and collaborate with healthcare providers to optimize patient care.
• Accurately document current and relevant patient information related to specialty medication therapy within the electronic health record (EHR).
• Complete and maintain initial patient assessments and ongoing reassessments in compliance with organizational policies and accreditation standards, including URAC requirements.
• Maintain thorough and accurate documentation of all patient interactions, including but not limited to: Telephone communications Emails In-person pharmacy visits Clinical consultations
• Ensure all documentation is timely, complete, and compliant with applicable regulatory and accreditation standards.
• Perform other duties and responsibilities as assigned to support departmental and organizational objectives.
