About The Position

This role focuses on clinical excellence and patient care within a hospital pharmacy setting. The Clinic Specialty Pharmacist will assess and optimize medication orders, oversee specialized dispensing and safety, collaborate with interdisciplinary teams, provide patient education, and adapt to dynamic clinical needs.

Requirements

Pharmacist credentialed from the Wisconsin Pharmacy Examining Board obtained prior to hire date or job transfer date required.
Bachelor's or advanced degree in Pharmacy required.
Doctor of Pharmacy
Minimum: 2 years of ambulatory care/chronic disease management experience.

Responsibilities

Assess, review, and optimize complex medication orders to proactively resolve drug-related issues, recommending formulary alternatives and serving as a subject matter expert for interprofessional care teams and clinical leadership.
Oversee the precise compounding, dosing, and dispensing of medications, ensuring rigorous safety standards across specialized patient populations.
Direct-consult with physicians, nurses, and care managers to streamline treatment plans and provide expert drug information to healthcare staff.
Empower patients through direct medication counseling, helping them safely navigate and understand their therapy regimens.
Adapt to dynamic clinical needs by providing flexible coverage across pharmacy units when required.
