About The Position

This position is for a Pharmacist Resident at SSM Health St. Mary's Hospital - Madison, focusing on developing clinical pharmacy practice skills consistent with American Society of Health System Pharmacists (ASHP) accreditation standards. The resident will work collaboratively with preceptors to optimize patient care by promoting rational, safe, and cost-effective drug therapy through the development and provision of clinical services in their area of expertise. Responsibilities include providing pharmaceutical care in various clinical practice areas, such as reviewing patient drug/medical history, offering drug therapy recommendations, providing drug information, evaluating medical literature, rounding with healthcare providers, conducting drug utilization reviews, patient counseling, and performing order entry/verification/dispensing.

Requirements

• Pharmacy degree (Pharm.D; Bachelor of Pharmacy) from an ACPE accredited school of pharmacy
• No experience required
• Basic Life Support Healthcare Provider (BLS HCP) required within 90 day of employment
• Other resuscitation certifications may be required dependent on department
• Pharmacist, Registered - Illinois Department of Financial and Professional Regulation (IDFPR) (for Illinois)
• Pharmacist - Missouri Division of Professional Registration (for Missouri)
• Pharmacist - Oklahoma State Board of Pharmacy (for Oklahoma)
• Pharmacist - Wisconsin Department of Safety and Professional Services (for Wisconsin)

Responsibilities

• Provides pharmaceutical care in the various areas of clinical practice consistent with his/her preceptor.
• Plans, implements, performs and measures the clinical processes that will improve treatment outcomes while minimizing adverse drug events and contributes to quality and financial outcomes.
• Participates in drug utilization evaluations (DUEs), analyzes trends and evaluates data to be synthesized into reports for action by committees (if applicable).
• Participates in a culture of learning and sharing that supports his/her development in the residency program.
• Implements staff development on pharmacy processes and participates in the training of other pharmacists, pharmacy residents, pharmacy students and healthcare providers as appropriate for the site.
• Promotes interdisciplinary teamwork including development and implementation of clinical pathways and protocols that promote appropriate medication safety and medication utilization using best practice, evidence-based recommendations.
• Ensures safety and appropriateness of medication therapy through order verification, dispensing, home medication reconciliation, chart review, patient counseling, lab value monitoring and dose/therapy adjustment as applicable.
• Documents medication related plans and interventions in electronic medical record.
• Oversees the medication dispensing process to ensure accurate and timely delivery of optimal pharmacotherapy to patients.
• Supervision of pharmacy technicians and/or interns who support the role of the pharmacist in the drug delivery system.
• Works in a constant state of alertness and safe manner.
• Performs other duties as assigned
