About The Position

The Pharmacist will play a vital role in our inpatient rehabilitation hospital, ensuring the safe and effective use of medications to optimize patient outcomes. This position involves collaborating with our interdisciplinary team of healthcare professionals to provide comprehensive pharmaceutical care, patient education, and drug therapy monitoring.

Requirements

  • Current, unencunbered license to practice as a Pharmacist in the state where currently practicing.
  • B.S. or PharmD from an ACPE-accredited School of Pharmacy
  • Strong clinical knowledge of pharmacology and drug therapy principles.
  • Organizes and prioritizes tasks and projects, accurately completing job responsibilities within the specified time constraints.
  • Excellent attention to detail and accuracy.
  • Analytical and problem-solving abilities.
  • Ability to work under deadlines and handle multiple tasks.
  • Speaks and communicates English effectively in verbal and written format; writes legibly.
  • Reading and comprehension at the level necessary to perform job duties appropriately.
  • Works cooperatively with co-workers and other hospital employees
  • Functions proficiently in a Microsoft Windows environment (Outlook, Word, Excel, PowerPoint)

Responsibilities

  • Review and verify medication orders for accuracy, appropriateness, potential drug interactions, and allergies.
  • Prepare, compound, and dispense medications according to established protocols and physician orders.
  • Provide clear, understandable information to patients and their caregivers regarding medication use, potential side effects, and safe administration practices.
  • Assess patient medication regimens for safety, efficacy, and potential adverse drug reactions. Collaborate with physicians to recommend adjustments or alternatives as needed.
  • Participate in multidisciplinary patient care rounds, providing medication-related expertise and recommendations to the healthcare team.
  • Maintain appropriate drug inventory levels, ensuring the timely ordering of medications and supplies while overseeing the proper storage and disposal of pharmaceuticals.
  • Adhere to all federal, state, and local regulations governing pharmacy practice, including controlled substance management and documentation.
  • Remain updated on the latest advancements in pharmacology and therapeutics through participation in professional development activities.
