About The Position

The Chinle Comprehensive Health Care Facility (CCHCF) is a 60-bed inpatient hospital and outpatient facility located in Chinle, Arizona, serving approximately 35,000 Navajo citizens. This facility provides a wide range of medical services to a rural population, many of whom travel significant distances for care and may have limited access to basic utilities. The role of the Outpatient Pharmacist involves performing direct patient care services, managing medication therapy, and ensuring accurate dispensing and patient counseling within the CCHCF's electronic health record system. The pharmacist will evaluate medication regimens, document patient care, and adhere to safety reporting requirements. This position serves patients of all ages in both outpatient and inpatient settings.

Requirements

Must be board certified.
Must be a graduate from a Pharmacist Educational program, accredited by the Accreditation Council for Pharmacy Education or its predecessors, at a college, university, or educational institution that is accredited by an accrediting body or organization recognized by the U.S. Department of Education at the time the degree was obtained.
Shall possess a current, valid, and unrestricted pharmacy license in a state, the District of the Commonwealth of Puerto Rico, or a Territory of the United States.

Nice To Haves

Shall have current BLS, ACLS, and/or PALS preferred.

Responsibilities

Perform pharmacy or direct patient care services.
Adhere to competency standards for outpatient pharmacy services at CCHCF.
Document patient care using CCHCF's electronic health record system.
Manage medication therapy, evaluate patient responses, make recommendations, and monitor care plans.
Gather patient health history, adjust treatment plans, and update clinical activities, problem lists, and medication allergy information in the EHR.
Evaluate medication regimens for appropriateness, interactions, compliance, and adverse events.
Accurately dispense prescriptions, counsel patients on medication use, storage, and side effects, and document provided cognitive services.
Adhere to safety reporting requirements outlined in CCHCF's Safety Measure Policy.
Rotate on-call shifts, provide drug information, process inpatient orders, prepare IVs, and restock medication dispensers.
Serve patients of all ages in outpatient and inpatient settings.
Review electronic health records to ensure appropriate prescriptions, verify accuracy, and counsel patients on proper medication use, dosage, and expected outcomes.
