About The Position

Community Regional Medical Center (CRMC) is the flagship hospital of Community Medical Centers – a not-for-profit healthcare system based in Fresno, California, and the largest healthcare provider and private employer in central California. CRMC encompasses several additional campuses totaling 909 beds to offer the most comprehensive care to our region which includes; the Fresno Heart and Surgical Hospital, a behavioral health center, a subacute and transitional care center, a cancer center, and home health services. CRMC is home to the only Level 1 Trauma Center and has the only comprehensive burn care in the Valley serving 2.5 million people within 9 counties. The 56,000 square-foot emergency department is one of the largest and busiest in the state. We have an 84 bed Level 3 NICU and a brand new pediatric specialty care unit. Future plans include building a women and children’s facility on campus and expanding with 65 additional beds. Requirements

  • Doctor of Pharmacy (PharmD) Degree from an ACPE-accredited institution required
  • PGY-1 Pharmacy Practice Residency or significant ambulatory clinical experience required
  • Current and unrestricted Registered Pharmacist (RPh) License in California required
  • Board certification (BCPS, BCACP, or equivalent) required within 18 months of hire
  • Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required

Nice To Haves

  • PGY-2 residency in ambulatory care or related specialty preferred
  • Advanced clinical expertise in chronic disease management preferred
  • Experience or familiarity with 340B program operations and compliance preferred
  • Demonstrated pharmacy practice experience in ambulatory or HOPD pharmacist-managed clinics preferred

Responsibilities

  • Provides comprehensive medication management services within HOPD referral-based pharmacist-managed clinics.
  • Initiates, modifies, and discontinues medication therapy under a CPA and in accordance with state regulations.
  • Conducts comprehensive patient assessments, including medication reconciliation and review of medical history under a CPA.
  • Orders, interprets, and monitors laboratory tests to ensure medication efficacy and safety under a CPA.
  • Manages complex and high-risk chronic disease states (such as oncology supportive care, weight loss, diabetes, hypertension, heart failure, anticoagulation, COPD and hyperlipidemia) under a CPA.
  • Develops and implements individualized, evidence-based treatment plans aligned with clinical guidelines under a CPA.
  • Monitors therapeutic outcomes and adjust pharmacotherapy to achieve optimal clinical goals under a CPA while identifying and resolving medication-related problems.
  • Provides patient education and counseling to improve medication adherence and support disease self-management through in-person or virtual visits.
  • Collaborates with referring providers, interdisciplinary teams, payers, retail pharmacies, and specialty pharmacies to coordinate care and address medication access issues.
  • Documents all patient encounters and clinical interventions in the electronic health record (EHR) and track clinical outcomes and quality metrics to support value-based care and quality improvement initiatives.
  • Serves as a preceptor for pharmacy students and residents when applicable.
  • Performs other job-related duties as assigned.
