About The Position

Under the supervision of a licensed Pharmacist, the Pharmacist Intern will ensure the safe and effective use of medications within HSN and learn the operational and clinical skills required to be a Pharmacist. This role reports under the general direction of the Clinical Manager, Pharmacy.

Requirements

  • Minimum of a four (4) year Bachelor’s Degree in Pharmacy, from an accredited university.
  • Current Certificate of Registration in good standing with the Ontario College of Pharmacist (OCP) is required.
  • Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.
  • Demonstrated knowledge of Pharmacy distributive services.
  • Demonstrated knowledge of electronic order entry processes.
  • Demonstrated computer proficiency in clinical systems.
  • Ability to interact with physicians and other healthcare providers in a professional and effective manner.
  • Demonstrated training, experience or utilization of quality improvement methodology for process improvement.
  • Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
  • Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
  • Demonstrated superior interpersonal and communication skills, both written and verbal.
  • Demonstrated effective time management and organizational skills with the ability to organize and prioritize as required.
  • Demonstrated discretion and maturity when handling confidential information.
  • Demonstrated commitment to the safety of co-workers and patients.
  • Successful criminal reference check is required for vulnerable sector.
  • Ability to use tact and discretion in dealing with health care providers and patients.
  • Proven ability to work independently and in a team environment.
  • Demonstrated ability to work effectively as a member of an interdisciplinary team.
  • Demonstrated ability to perform with minimal supervision; to prioritize duties.
  • Demonstrated professionalism in dealing with confidential and sensitive issues.

Nice To Haves

  • Doctorate of Pharmacy (PharmD) is preferred.
  • Experience working as a Pharmacy Student within a hospital environment is preferred.
  • Experience working in community pharmacy is preferred.

Responsibilities

  • Provide clinical pharmaceutical care to patients.
  • Provide oral and written follow-up with treatment recommendations to the prescriber as required.
  • Provide medication reconciliation services and interact with patients/families and other health care professionals with regards to patients’ home medication usage.
  • Ensure completion of required documentation in conformance with the standards of practice of the Ontario College of Pharmacists (OCP) and HSN policies.
  • Interact with health care professionals and the patient/family regarding the patient’s plan of care; handle drug-related problems.
  • Ensure verification of all technical work by directly confirming or via to the standards of practice of the OCP and HSN policies.
  • Perform Therapeutic Drug Monitoring (TDM) for high risk drugs.
  • Maintain compliance with departmental policies and procedures.
  • Complete departmental projects as required.
  • Engage in professional self-development.
  • Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
  • Contribute to learner development by providing academic support, mentorship, preceptorship and supervision as required.
  • Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
  • Educate and promote health, safety and wellness in the work place.
  • Represent the department or program on various committees and in meetings as required.
  • Perform other duties as required.
