About The Position

This position fulfills the duties of the Pharmacist-in-Charge (PIC) as outlined by the Oregon Board of Pharmacy (OAR 855-019-0300). It provides pharmaceutical care as a complete package across the hospital medication process to increase the effectiveness and quality of patient care. This position does not directly manage any other caregivers. Pharmacists may be asked to work at multiple St. Charles Health System worksites at the request of leadership to fulfill short term, immediate staffing needs.

Requirements

Bachelor of Science degree in Pharmacy.
Current registration by the Oregon Board of Pharmacy.
AHA Basic Life Support for Healthcare Provider certification.
Minimum one (1) year of pharmacy practice.
Completion of OBOP approved PIC training course within first year of appointment.
Must be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely.

Nice To Haves

PharmD.
Completion of an ASHP accredited PGY-1 residency training.
Previous PIC experience.
Any certification through the Board of Pharmacy Specialties (BPS).

Responsibilities

Assists with design, deployment, and sustainment of Oregon Board of Pharmacy standards in collaboration with the CPO and pharmacy leadership.
Participates in and represents pharmacy leadership at huddles, committees, and work groups as assigned.
Maintains alignment with pharmacy system goals and initiatives.
Coordinates and oversees students and preceptor duties for the pharmacy.
Provides specialized pharmaceutical services to improve drug usage and therapeutic outcomes, including advising physicians on drug therapy, toxicity, side effects, and appropriate doses.
Coordinates the activities of assigned pharmacy technicians in the preparation, labeling, and delivery of medications and pharmaceutical supplies, ensuring compliance with laws, regulations, and record-keeping.
Mentors and trains others, including practical experiences.
Maintains liaison relationships with medical and nursing staff, providing information on pharmaceutical supplies, drug usage, compatibility, drug control regulations, and Joint Commission Standards.
Conducts and evaluates medication histories, assesses compliance, suggests modifications, and instructs patients on proper drug use. Participates in Patient Care Rounds with physicians.
Individualizes medication regimens using sound principles, accounting for pharmacodynamic and pharmacokinetic variations, and is responsible for establishing and improving the delivery of Pharmaceutical Care.
Extracts pertinent information from patient/other sources for the patient's medical management plan.
Maintains flexibility and readiness to work in multiple worksites.
Operates pharmacy computer system(s) and other equipment.
