The Clinical Pharmacy Specialist - Heart Failure is responsible for providing comprehensive pharmaceutical care to patients through clinical services, formulary management, development of policies and procedures, and integrated drug distribution. Individual will be responsible for providing comprehensive medication management (primarily to heart failure patients at high risk for readmission) through clinical services, patient and provider education, and assisting with ensuring safe medication use on the patients' transition of care (i.e., discharge) from the hospital. Individual may also be responsible for providing recommendations and/or assuming responsibility for patient care activities such as pharmacokinetics, anticoagulation, parenteral nutrition support, or other medication regimens as prescribed by provider.

Requirements

  • Bachelors Pharmacy or Doctorate Pharmacy
  • RPH - Reg Pharmacist
  • Minimum 3 years clinical pharmacy experience or PGY1 Pharmacy Residency completion
  • Minimum 2 years clinical pharmacy experience in cardiology and/or management of heart failure patients or completion of a PGY2 Cardiology Residency
  • Strong verbal and written communication skills
  • Strong computer skills.
  • Capable of preparing and providing presentations
  • Strong organizational skills

Nice To Haves

  • Board Certification (e.g., BCPS, BCCP, etc.)
  • Pharmacist Registration in the state of Georgia within 180 Days
  • Experience with nutrition support, anticoagulation management, and pharmacokinetics
  • Experience with Epic, Microsoft Office

Responsibilities

  • Provide recommendations and/or manage therapy associated with heart failure medications, anticoagulation, antimicrobial stewardship/pharmacokinetics, parenteral nutrition or other medication regimens as prescribed by the provider.
  • Document clinical interventions on an ongoing basis.
  • Round with interdisciplinary teams.
  • Clarify medications upon admission and discharge, as needed.
  • Optimize medication selection to improve medication access post-discharge.
  • Demonstrate a basic understanding of distributive responsibilities including computer functions.
  • Assist in investigational drug studies when needed.
  • Provide on-call clinical pharmacy services as needed.
  • Counsel patients on their discharge medications/perform MTM services.
  • Develop and provide educational programs/inservices for health care practitioners and patients as required.
  • Provide drug information when needed or requested.
  • Serve as a preceptor/instructor for students from accredited colleges/schools of pharmacy.
  • Serve as a preceptor for the Pharmacy Residency Program (where applicable).
  • Assist in developing drug protocols.
  • Participate as a member of the WellStar Pharmacy and Therapeutics (P&T) Committee, as well as other System and facility-specific committees as assigned or as deemed necessary.
  • Identify target medications/therapies, develop criteria, and implement data collection for Medication Use Evaluations (MUE) or other heart failure-related studies/initiatives.
  • Tabulate and present completed MUE data to the P&T committee.
  • Monitor metrics related to heart failure patients and develop improvement strategies as necessary.
  • Assist in formulary maintenance by critiquing new entities, comparing medication classes, and developing therapeutic interchanges where applicable.
  • Assist in preparing for and maintaining compliance with regulatory agency (e.g., The Joint Commission) standards/requirements.
  • Performs other duties as assigned.
