About The Position

This is a full-time, day shift Pharmacist position for St. Clare Hospital in Baraboo, working a total of 40 hours per week. The role involves providing pharmaceutical care to patients, verifying medication orders, compounding and dispensing pharmaceuticals in accordance with regulations and hospital policy. The pharmacist will serve as a resource to other healthcare professionals, including pharmacy students and residents, ensuring patient care meets standards of safety and effectiveness. This position also oversees the duties of pharmacy technicians and interns.

Requirements

  • Pharmacy degree (Pharm.D; Bachelor of Pharmacy) from accredited school of pharmacy
  • No experience required
  • Basic Life Support Healthcare Provider (BLS HCP) required within 90 days of employment.
  • Other resuscitation certifications may be required dependent on department.

Responsibilities

  • Ensures safety and appropriateness of medication therapy through order verification, dispensing, home medication reconciliation, chart review, patient counseling, lab value monitoring and dose/therapy adjustment as applicable.
  • Documents medication related plans and interventions in electronic medical record.
  • Provides clinical consultation delivering best-practice, evidence-based and complete information to practitioners.
  • Makes recommendations and encourages safe and cost-effective pharmacotherapy for patients through multi-disciplinary collaboration.
  • Oversees the medication dispensing process to ensure accurate and timely delivery of optimal pharmacotherapy to patients.
  • Supervises pharmacy technicians and/or interns who support the role of the pharmacist in the drug delivery system.
  • Precepts pharmacy students and residents, as applicable.
  • Works in a constant state of alertness and safe manner.
  • Performs other duties as assigned.
