About The Position

This is a part time variable shift Clinical Pharmacist position for St. Agnes Hospital in Fond du Lac working a total of 24 hours per week. The position involves providing pharmaceutical care to patients, verifying medication orders, compounding and dispensing pharmaceuticals in accordance with regulations and hospital policy. The pharmacist will serve as a resource to other healthcare professionals, including pharmacy students and residents, ensuring patient care meets standards of safety and effectiveness. This role also includes overseeing the duties of pharmacy technicians and interns.

Requirements

  • Pharmacy degree (Pharm.D; Bachelor of Pharmacy) from accredited school of pharmacy
  • Basic Life Support Healthcare Provider (BLS HCP) required within 90 day of employment.
  • Other resuscitation certifications may be required dependent on department.
  • Pharmacist, Registered - Illinois Department of Financial and Professional Regulation (IDFPR)
  • Pharmacist - Missouri Division of Professional Registration
  • Pharmacist - Oklahoma State Board of Pharmacy
  • Pharmacist - Wisconsin Department of Safety and Professional Services

Responsibilities

  • Ensures safety and appropriateness of medication therapy through order verification, dispensing, home medication reconciliation, chart review, patient counseling, lab value monitoring and dose/therapy adjustment as applicable.
  • Documents medication related plans and interventions in electronic medical record.
  • Provides clinical consultation delivering best-practice, evidence-based and complete information to practitioners.
  • Makes recommendations and encourages safe and cost-effective pharmacotherapy for patients through multi-disciplinary collaboration.
  • Oversees the medication dispensing process to ensure accurate and timely delivery of optimal pharmacotherapy to patients.
  • Supervises pharmacy technicians and/or interns who support the role of the pharmacist in the drug delivery system.
  • Precepts pharmacy students and residents, as applicable.
  • Works in a constant state of alertness and safe manner.
  • Performs other duties as assigned.
