About The Position

The Georgia Department of Public Health (DPH) is seeking a compassionate and dedicated Public Health Nurse to join our Home Visiting Program. This role involves providing professional nursing care, case management, and health education to pregnant women, postpartum mothers, and infants through home visitation services. The program aims to improve maternal and child health outcomes and promote healthy families. Services are delivered primarily in community settings through frequent home visits to an assigned caseload. Extensive travel throughout the Northeast Health District's 10-county service area is required. The primary work location and service area may be determined based on the employee's residence to best support program operations and community needs.

Requirements

  • Associate's degree in nursing from an accredited college or university
  • Completion of a PHN course within one year of hire
  • Current Georgia Registered Professional Nurse license
  • CPR certification
  • A valid driver's license and reliable transportation are required.

Responsibilities

  • Provide professional nursing care and case management in accordance with established public health protocols, policies, and procedures.
  • Promote maternal and child health through health education, illness prevention, and interventions designed to reduce infant mortality.
  • Conduct home visits and community outreach to high-risk perinatal women, postpartum mothers, and infants.
  • Perform nursing assessments, obtain vital signs, conduct fetal monitoring, and perform basic laboratory screenings.
  • Provide breastfeeding education, counseling, and referrals to appropriate medical, behavioral health, and community resources.
  • Maintain accurate and timely documentation of patient care, services provided, and client progress.
  • Collaborate with physicians, healthcare providers, community agencies, and multidisciplinary team members to coordinate care and improve client outcomes.
  • Participate in emergency preparedness and public health response activities as assigned.
  • Perform other duties as assigned.
