About The Position

This 12-month, post-graduate year two training program builds on PGY1 residency experiences to contribute to the development of pharmacists who will specialize in clinical or administrative pharmacy practice. The program provides residents with opportunities to function independently as practitioners by conceptualizing and integrating accumulated experience and knowledge and incorporating both into the provision of patient care or other advanced practice settings. Residents who successfully complete this PGY2 program are prepared for advanced patient care, academic, or other specialized positions, along with board certification, if available. The residency program’s purpose is achieved by utilizing the four competency areas and their associated educational goals and objectives as required by the American Society of Health System (ASHP) Standard. The competency areas are: (1) patient care; (2) advancing practice and improving patient care; (3) leadership and management; and (4) teaching, education, and dissemination of knowledge.

Requirements

  • PharmD from an ACPE accredited program.
  • Completed an ASHP-accredited PGY-1 Residency.
  • Unrestricted Pharmacist license within the first 90 days of employment.

Nice To Haves

  • Experience in drug utilization review and analysis.
  • Experience with Electronic Medical Records.

Responsibilities

  • Rotates though various areas of the Department of Pharmacy appropriate to the specialty for which the program was designed (e.g., critical care, cardiology, specialty pharmacy health system management).
  • Reviews and approves medication orders for all Medical Center patients. This includes weekend, holiday, evening coverage as described in departmental policies and procedures.
  • Provides specialized continuing education for professional staff (e.g., pharmacists, physicians, nurses). Educates other health care providers by publishing drug information articles in the pharmacy newsletter.
  • Participates in periodic reviews of therapeutic classes of drugs and present findings to the Pharmacy and Therapeutics Committee.
  • Prepares and presents to the appropriate committee’s evidence-based reviews of drugs requested for formulary addition.
  • Initiates and facilitates completion of drug use evaluations.
  • Detects and reports medication errors and adverse drug reactions in a timely manner.
  • Provides accurate, adequate and timely drug information to Tufts Medical Center professional staff.
  • Conducts medication history interviews, reconcile patients’ medication therapies and provide drug information to patients and their families.
  • Coordinates specialized care to ensure safe and effective transitions from inpatient to outpatient care.
  • Documents in the medical record patient evaluations and recommendations, medication reconciliation and medication histories.
  • Documents pharmacist interventions according to established policies.
  • Participates in a number of longitudinal activities including and completes of a year-long residency project appropriate to the specialty for which the program was designed.
