About The Position

This is a 12-month first-year postgraduate training program. The resident will be a self-directed, independent, motivated learner guided by the Residency Program Director and the appropriate preceptors for each learning experience. The resident will rotate through 12 months of inpatient and ambulatory patient care experiences to foster independent clinical decision making through established collaborative drug therapy management agreements. Primary clinical responsibilities of the resident include rounding with assigned teams, performing pharmacokinetic monitoring, validating pharmacy orders, performing medication reconciliation, ensuring appropriate transitions of care, and providing patient education. Ultimately, the resident is responsible for providing comprehensive medication therapy management for patients by designing, recommending, and monitoring therapeutic regimens that incorporate evidence-based medicine.

  • Must be able to collaborate with physicians, medical residents, and other prescribers
  • Must be able to provide drug information to healthcare professionals
  • Must be able to monitor for medication efficacy and toxicity
  • Must be able to provide therapeutic drug level monitoring
  • Must be able to administer medication reconciliation and drug discharge counseling
  • Must be able to provide pharmacokinetic, pharmacotherapeutic, infectious diseases, and pharmacy-related consultations
  • Must be able to detect, report, and manage adverse drug events and medication errors
  • Must be able to document pharmacotherapy interventions
  • Must be able to conduct a medication-use evaluation and research project
  • Must be able to present regularly to clinical pharmacy staff at Pharmacotherapy Grand Rounds
  • Must complete regular evaluations in PharmAcademic
  • Must earn faculty appointment at Touro College of Pharmacy
  • Must serve as covering clinical pharmacist and/or staff pharmacist on roughly one evening per week and every third weekend
  • Must work professionally and respectfully and build relationships with each multidisciplinary team

Responsibilities

  • Participates in daily medical and pharmacy teaching rounds and provides patient care pharmacotherapy services
