Summary : The pharmacy resident PGY1 is a part of a 12-month, structured, experiential training program with responsibilities including staffing as a pharmacist as well as participating in a variety of intradepartmental and interdepartmental projects and activities. The pharmacy resident performs a combination of clinical and distributive pharmacy services necessary for the provision of high-quality pharmaceutical care to ThedaCare patients. The pharmacy resident is part of the pharmacy team at ThedaCare and therefore the resident is expected to maintain flexibility and be willing to support the department in times of unanticipated need. Other duties as assigned.

Requirements

  • PharmD degree from an ACPE accredited school of pharmacy
  • Good understanding of pharmacology and therapeutics, acquired through a combination of school and clerkship experience, in order to recommend appropriate and patient specific therapeutic regimens and subsequently monitor outcomes.
  • Ability to communicate effectively with other health care professionals, administrators, and patients.
  • Ability to prioritize multiple responsibilities.
  • Working knowledge of Microsoft computer programs including, Word, Excel, Outlook, and PowerPoint.
  • Must obtain American Heart Association Healthcare Provider Basic Life Support (BLS) and American Heart Association Advanced Cardiac Life Support (ACLS) certification within 120 days of hire.
  • Wisconsin pharmacist license (or eligible for) is required within 120 days of hire with ability for extension to be granted per policy.

Responsibilities

  • Work toward accomplishing and becoming proficient as a clinical pharmacist as outlined in the PGY 1Pharmacy Residency Goals &Objectives.
  • Staff on-line as a pharmacist as scheduled and needed to optimize patient care. (see Clinical Pharmacist Job Description)
  • Maintain patient confidentiality.
  • Conduct medication histories and reconciliation at admission.
  • Instruct patients on the proper use of their medications.
  • Provide drug information for pharmacists, nurses, and physicians.
  • Promote safe and cost-effective physician prescribing.
  • Participate in the education of pharmacy and medical students, interns, and residents.
  • Communicate effectively with colleagues as needed to enhance high quality and efficient patient care.
  • Participate in the medication use evaluation (MUE) process to assist the hospital in optimizing drug therapy.
  • Identify and report potential adverse drug reactions to enable department to monitor trend, anticipate future adverse reactions, and subsequently improve patient care.
  • Identify and report medication errors as well as participate in error investigation to enhance patient safety.
  • Attend pharmacy relevant meetings per direction of pharmacy preceptors.
  • Prepare and deliver informal and formal presentations to a variety of groups.
  • Complete activities as outlined in individual rotations and learning experiences.
  • Complete all assignments on a timely basis.
  • Represent institution at various local, regional, and national meetings and conferences.
  • Prepare and present a poster at the ASHP Midyear meeting.
  • Prepare and present a presentation at the Wisconsin Pharmacy Residency Conference (WPRC)
  • Actively participate in the recruitment of future residents.
  • Participate in various quality improvement projects
