About The Position

The LOFT Assisted Living Hub and Spoke Orillia program provides specialized support to vulnerable and at-risk older adults with complex physical health challenges, mental health challenges, addictions or cognitive impairment. The program supports each client’s individual needs so they can live safely and in good health in the community as they continue to age. The program helps provide stability to individuals in the surrounding community, helps reduce unnecessary or avoidable ED visits and reduces the length of stay in hospital. The program works with each client on an individualized client care plan based on their wants, needs, and strengths. Additionally, LOFT staff utilize a standardized assessment tool called the RAICHA to highlight and mitigate risk for decline in the client’s health. The Program Coordinator and Psychogeriatric Case Manager work collaboratively with clients to identify and mitigate barriers that affect their physical and mental health, integrating social determinants of health into their practice. The LOFT Assisted Living Hub and Spoke Orillia program is a community-based program that provides care coordination, case management and personal support. The team can also provide access to specialized services for mental health and addictions and emergency response planning and equipment. Services may be provided up to 5 hours a week for as long as each client requires support. The Personal Support Worker (PSW) is responsible for rendering a range of services for the clients of the program consistent with its basic values and goals. The PSW will support, as assigned, the needs of the vulnerable, at risk, older adults and seniors living in the multi-sites under the auspices of the program. This support relates to the physical, cognitive, mental health and social/recreational needs of the clients. The staff member will maintain the established routines of the program including all security, fire and general health and safety practices. The PSW will perform these and other delegated responsibilities to ensure the quality of personal care of the clients of the program.

Requirements

  • Post-secondary education in Human Services or a related field (e.g., PSW Certificate, Gerontology Diploma).
  • Experience supporting seniors and vulnerable populations, including individuals with cognitive and mental health needs.
  • Knowledge of medication administration, health conditions, and psychiatric care.
  • Current First Aid and CPR certification.
  • Strong skills in assessment, problem-solving, and crisis intervention.
  • Ability to engage clients in a positive, respectful, and supportive manner.
  • Ability to work collaboratively within a multidisciplinary team.
  • Effective verbal and written communication skills.
  • Demonstrated creativity, flexibility, and adaptability.
  • Proficiency with basic computer applications.
  • A satisfactory vulnerable sector check (VSC) is also required.

Nice To Haves

  • Experience coordinating social and recreational activities is an asset.
  • A valid Ontario Driver’s License is preferred.

Responsibilities

  • Provide compassionate support for clients’ physical and mental health needs, including medication reminders, health monitoring, and coordination with healthcare professionals.
  • Assist with personal care, hygiene, mobility, meal preparation, and maintaining a clean, safe, and comfortable living environment.
  • Build respectful relationships that promote dignity, independence, and active participation in daily, social, and recreational activities.
  • Assist clients in managing behavioural and cognitive challenges, offering guidance, reassurance, and companionship.
  • Ensure a safe environment by following health and safety procedures, responding to emergencies, and reporting incidents appropriately.
  • Follow established medication protocols, support self-administration programs, and collaborate with healthcare providers to ensure safe practices.
  • Maintain accurate care records, participate in care planning, and communicate effectively with the interdisciplinary team.
  • Assist with program activities, transportation for client outings, and day-to-day operations, including basic administrative tasks when required.
  • Work rotating shifts, provide coverage as needed, and support overall program operations, including during the absence of senior staff.
  • Additional responsibilities may be assigned by the Program Manager and/or designated supervisor to support overall program operations and continuity of care.
