About The Position
We are seeking a dependable and caring Personal Care Specialist (CNA) to provide hands-on support to patients in their homes while working closely with nurses and the interdisciplinary hospice team. As a Personal Care Specialist, you'll help patients remain comfortable and maintain their dignity and independence in the comfort of their own homes.
Requirements
- High school diploma or GED required
- Basic computer skills and ability to use electronic documentation systems
- Valid driver's license and safe driving record
- Reliable transportation and state-required automobile insurance
- Ability to travel locally between patient homes
- Ability to work in a variety of weather and driving conditions
Nice To Haves
- Current Ohio STNA/CNA certification preferred
- At least one year of healthcare or direct patient-care experience preferred
- Hospice certification (CHPNA) is encouraged within two years of employment
Responsibilities
- Assist patients with activities of daily living, including bathing, dressing, grooming, toileting, and mobility
- Provide compassionate, respectful, and patient-focused care
- Observe patients for changes in condition and promptly report concerns to the appropriate member of the care team
- Accurately document care and services provided using electronic systems
- Promote patient safety, comfort, privacy, and dignity
- Communicate effectively with nurses, caregivers, family members, and other members of the hospice team
- Develop trusting relationships with patients and their loved ones
- Follow the individualized plan of care and hospice policies and procedures
