About The Position

Join our team as a QMAP Personal Care Partner II and make a real difference in residents' lives! Requirements

  • Medication certification (QMAP-A/L, Med Tech, Med Aide, Nurse Delegation) per state requirements.
  • Ability to read, write, and speak the English language.
  • High school or Equivalent (GED).
  • Proficient computer skills.
  • Strong self-management and organization skills.
  • Strong multi-tasking abilities with the ability to handle competing deadlines, flexible and adaptable.
  • Ability to effectively handle difficult and sensitive issues and maintain confidentiality.

Nice To Haves

  • One (1) year of experience in elder care.

Responsibilities

  • Respond promptly to residents' needs and assist with daily activities like bathing, dressing, meals, and laundry.
  • Spot and report care needs before they become significant issues, such as hygiene concerns, broken glasses, skin changes, or missing hearing aids.
  • Communicate with supervisors and teammates to stay informed about resident needs.
  • Monitor resident well-being and contribute to a secure and comfortable environment.
  • Report any concerns about abuse, neglect, or misuse of resources.
  • Stay alert to changes in resident health or status and inform supervisors.
  • Document resident conditions as directed.
  • Support residents with their daily medication routine in compliance with state regulations, and flag any questions or concerns with the RN/LPN team.
  • Keep medication records organized and up-to-date, including documenting any medication refusals or treatment changes.
  • Restock medication station supplies such as gloves and sanitizer.
  • Submit work orders for maintenance needs.
  • Attend monthly staff meetings to provide insights on resident and departmental needs.
  • Take on additional duties as needed to support the team and residents.
