About The Position

The primary purpose of this job position is to provide each of your assigned residents with routine daily care and services in accordance with the resident’s assessment and care plan, and as may be directed by your supervisors. This role involves ensuring resident well-being, maintaining a safe and comfortable environment, and accurately documenting care provided. The position requires adherence to established policies and procedures, cooperation with inter-departmental personnel, and effective communication with residents, supervisors, and other staff.

Requirements

  • High school education or its equivalent.
  • Proficiency in Microsoft Suite products.
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to effectively present information and respond to questions from managers and employees.
  • Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • Must occasionally lift and/or move up to 25 pounds.
  • Prolonged use of a desktop or laptop computer.
  • Regularly required to sit, stand, walk and talk, read or hear.
  • Frequent use of all office-related equipment to include; copier/scanner/fax, telephone, and calculator.

Responsibilities

  • Provide routine daily care and services to assigned residents according to their assessment and care plan.
  • Assist residents with Activities of Daily Living (ALDS), including meals, personal hygiene (dental and mouth care, bathing, hair care, dressing/undressing), and toileting.
  • Ensure resident rooms are clean, comfortable, and ready for occupancy, including making beds and changing linens.
  • Transport residents to different areas within the facility or to/from vehicles as needed.
  • Maintain a warm, positive, and calm atmosphere for residents.
  • Report any changes in resident condition, accidents, incidents, complaints, or grievances to the appropriate supervisor (Health and Wellness Director, Executive Director, Nurse Supervisor/Charge Nurse).
  • Maintain the confidentiality of all resident care information and report any suspected unauthorized access or disclosure.
  • Perform only those care tasks for which training has been received and use only trained equipment.
  • Assist with resident admissions, transfers, and discharges, including inventorying and storing personal possessions.
  • Assist with preparing residents for medical tests, examinations, and therapies.
  • Perform restorative and rehabilitative procedures as instructed.
  • Assist with the care of end-of-life residents and post-mortem care.
  • Maintain intake and output records and monitor bowel and bladder functions.
  • Provide eye and ear care as instructed.
  • Weigh and measure residents and record vital signs (TPRs) as instructed.
  • Ensure residents who are unable to call for help are checked frequently and answer calls promptly.
  • Follow established policies concerning exposure to blood/body fluids and report occupational exposures.
  • Report defective equipment to the Nurse Supervisor/Charge Nurse.
  • Honor resident refusal of treatment requests and report them to supervisors.
  • Assist with shaving male residents and maintaining hair cleanliness for female residents as instructed.
  • Utilize protective and adaptive equipment as instructed.
  • Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift.
  • Meet with nursing personnel on a regularly scheduled basis to assist in identifying and correcting problem areas, and/or the improvement of services.
  • Participate in and receive the nursing report as instructed.
  • Follow work assignments and schedules in completing assigned tasks.
  • Cooperate with inter-departmental personnel and other facility personnel.
  • Notify the facility when late or absent from work.
  • Report all allegations of resident abuse and/or misappropriation of resident property.
