About The Position

The personal care assistant/worker is responsible for providing various care services necessary for attending to the personal needs, support, and health status of participants. This position will primarily consist of home care assignments. However, occasional facility work will also be required. The position will require working one weekend a month and rotating holiday coverage.

Requirements

  • High School Diploma or equivalent required.
  • Minimum of one (1) year of documented experience working with a frail or elderly population or if less than one (1) year of experience, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.
  • Must be action-oriented, manage conflict well, be customer focused, have high decision quality and flexibility to adapt to ongoing change.
  • Ability to work with minimal supervision and exercise independent judgment.
  • Superior written and verbal communication and presentation skills appropriate for audience comprehension.
  • Valid drivers license and use of personal vehicle for home care visits.
  • Excellent organizational skills and the ability to perform multiple duties and functions related to daily operations while maintaining excellent customer service.
  • Position may require 10% travel within the Centers geographic region and use of personal vehicle.

Nice To Haves

  • Experience in a long-term care facility or with a community-based geriatric program preferred.

Responsibilities

  • Engage in ongoing participant assessment and monitoring and report on services provided and the self-care abilities of participant.
  • Provide personal care assistance with activities of daily living, including toileting, showers, eating, and grooming.
  • Assist in the maintenance of physical activity, exercise, and walking.
  • Identification of safety hazards and emergency situations and initiating corrective action as able.
  • Assist with general activities such as conversing with participants, recreation activities, meal and snack serving, and housekeeping.
  • Assist in identifying signs and symptoms indicating physical, emotional, or behavioral change in participants and reporting these signs to the center staff and/or supervisor.
  • Escort participants to appointments as necessary.
  • Assist transportation staff with loading and unloading of participants from vehicles. Assist participants being transported in PO vehicles as needed.
