About The Position

This role impacts participants lives by providing essential personal care services and health support. The position involves both home care and facility work. The Personal Care Assistant will conduct ongoing participant assessments and monitoring, provide assistance with daily activities such as toileting, grooming, and meals, and aid in maintaining physical activity and exercise routines. Responsibilities also include identifying safety hazards, responding to emergencies, assisting with recreational activities, meal service, and housekeeping, reporting any changes in participant's condition to center staff, and escorting participants to appointments and assisting with transportation. Some travel within the region may be required.

Requirements

• High School Diploma or equivalent.
• Minimum of one (1) year of documented experience working with a frail or elderly population or if less than one (1) year of experience, must receive appropriate training from the PACE organization upon hiring.
• Strong communication and organizational skills.
• Valid driver's license, insurance and use of personal vehicle.
• Ability to work independently and adapt to change.

Nice To Haves

• Preferred experience in long-term care or geriatric programs.
