The Certified Home Health Aide (CHHA) will be responsible for providing or assisting with activities of daily living (ADL), including personal hygiene, grooming, mouth, skin, and nail care. This role involves assisting with toileting, positioning, transfer activities, and frequent turning and repositioning of bed-bound residents/patients. The CHHA will also assist with prescribed exercises and range of motion (ROM) activities as taught by the Hospice Nurse, perform light housekeeping in the resident's personal unit (including changing bed linens and patient laundry), assist with feeding, and obtain vital signs (temperature, pulse, respirations) under the supervision of the Hospice Nurse. Additionally, the CHHA must report any changes in the resident's/patient's condition to the Hospice Nurse and/or facility nurse, documenting this information accurately. The role also includes assisting or reminding patients to take medication in compliance with regulatory standards.
