About The Position

This role impacts participants lives by providing essential personal care services and health support. The role includes both home care and facility work. Must be able to report at 7:30 am for home care assignments. Must be available to work 2 holidays per year. Mercy LIFE Sharon Hill, part of Trinity Health PACE, provides high-quality care to seniors in the communities we serve. Our interdisciplinary team offers comprehensive services, allowing seniors to remain independent at home. Requirements

High School Diploma or equivalent.
Minimum of one (1) year of documented experience working with a frail or elderly population or if less than one (1) year of experience, must receive appropriate training from the PACE organization upon hiring.
Strong communication and organizational skills.
Valid driver's license, insurance and use of personal vehicle for home care assignments.
Ability to work independently and adapt to change.

Nice To Haves

Preferred experience in long-term care or geriatric programs.
Some travel within the region may be required.

Responsibilities

Conduct ongoing participant assessments and monitoring.
Provide assistance with daily activities, including toileting, grooming, and meals.
Aid in maintaining physical activity and exercise routines.
Identify safety hazards and respond to emergencies.
Assist with recreational activities, meal service, and housekeeping.
Report any changes in participant's condition to center staff.
Escort participants to appointments and assist with transportation.
