About The Position

This is a Home Care position for a Personal Care Assistant II at Golden Valley Health Centers Central Valley PACE Center in Stockton. The role involves delivering patient care activities and tasks as assigned to assist nursing staff, acting as a mentor to other PCAs, and performing various duties including COVID and Influenza testing, delivering Durable Medical Equipment (DME), and assisting with Home Safety Assessments. The position requires travel throughout San Joaquin County. Initially, the role will be based and trained at the Modesto center until the Stockton location is operational. The schedule is Monday - Friday, 8am - 5pm, with flexibility for weekend work when needed.

Requirements

• Valid California Driver’s License, acceptable driving record and vehicle insurance.
• Must have reliable transportation.
• Must be able to treat people of all ages including geriatrics.
• Meet a standardized set of competencies for the specific position description established by Central Valley PACE and approved by CMS before working independently.
• High School Diploma or equivalent required.
• Current unencumbered CA licensure as a Certified Nurse Assistant, Home Health Aide, Emergency Medical Technician, Advanced Emergency Medical Technician or Paramedic; or certification from an accredited program for clinical medical assisting.
• Current BLS CPR Card certified by the American Heart Association
• Prior personal care assistance experience, or ability to pass internal competencies within six (6) months of hire.
• Ability to interact professionally and respectfully with geriatric individuals including those with cognitive decline and/or physical frailties.

• Assist with Home Safety Assessments when assigned only when there is down time on the schedule.

Responsibilities

• Acts as the mentor to PCA I.
• Communicates patient care needs to the LVN, supervising RN and update Manager.
• Assists in training PCA’s and other staff.
• Perform COVID and Influenza test when assigned.
• May assist in Day Center and Activities departments when there is down time.
• Deliver DME to PRT’s home and document in EMR.
• Assist with Home Safety Assessments when assigned only when there is down time on the schedule.
• Provides basic personal care assistance and assistance with any other activities of daily living across settings to assigned participants.
• Communicates effectively with participants, families, staff, other departments and agencies.
• Works effectively in a team setting.
• Documents treatments/procedures, observations, nursing care and patient activities on appropriate forms.
• Participates in teaching/learning process with others.
• Follows guest relations policies and procedures.
• Abides by all established policies, rules and regulations.
• Attends staff meetings and takes part in participant care planning.
• Observe each participant for any change in physical, mental, emotional and social functioning and shall report such changes to the licensed nurse.
• Performs patient care activities appropriately, reporting any circumstances or change in patient’s condition.
• Other projects and duties as assigned.
