Provides or assists clients in meeting needs for activities of daily living. Requirements

  • HeartSaver AED
  • Valid California Drivers License. Must provide DMV report prior to offer being made. No driving infractions 5 years prior to hire or during time in this job. Infractions include DUI, Suspended or Revoked License, Reckless Driving
  • Completion of a Certified Nursing Assistant class, Medical Assistant class, Home Health Aide class, or one semester of LVN or RN in recent past.

Nice To Haves

  • Previous experience preferred.

Responsibilities

  • Assists client/patient with personal care according to the care plan to achieve established goals.
  • Assists with toilet needs and ambulation and transfer of the patient/client.
  • Supports the patient/client's nutrition through meal planning, purchasing groceries, preparing and serving food according to dietary needs.
  • Provides household services deemed necessary to maintain a safe, hygienic environment.
  • Performs special procedures when formally trained to do so, under the direction and authority of the supervisor, within scope of practice.
  • Reports pertinent information, including changes in condition/problems to the supervisor.
  • Maintains continuity of care through the support of the home care team.
  • Provides exceptional level of care with compassion and sensitivity to those nearing the end of life’s journey.
  • Demonstrates the knowledge and skills necessary to provide care and services appropriate to the population served on the assigned unit or work area.
  • Knowledgeable of growth and development for all patient/family cultural, linguistic, spiritual, gender, and age specific needs.
  • Able to effectively communicate and care for patient and family as reflected in the Plan for Provision of Care.
  • Performs other duties as assigned.
