This full-time position provides assessment, linkage, and advocacy services to individuals in the Permanent Supportive Housing (PSH) program who wish to receive housing case management services that promote individual empowerment and reduce the need for more intensive services. The case manager will collaborate closely with individuals, families, and other service providers to develop and implement individual service plans focused on housing stability. This provider will be responsible for building rapport and maintaining positive, professional relationships with landlords in the community. Permanent Supportive Housing services will be provided to clients who are experiencing chronic homelessness, have been diagnosed with a Serious Mental Illness, and are frequent users of emergency services. SOAR certified or applicants that are eligible to become certified are encouraged to apply.
