About The Position

Access Health Lakelands is seeking a Perinatal Nurse Coordinator to join their team. This full-time role operates on a first shift schedule, Monday through Friday, from 8:00 AM to 4:30 PM. The coordinator will be responsible for identifying and managing high-risk patients, conducting clinical chart audits, supporting remote patient monitoring, and providing direct patient education and coordination. The nurse will work collaboratively within a multidisciplinary team to address both medical and social needs, ensuring comprehensive care for expectant mothers across Self Regional Healthcare’s service area. This role may also involve conducting home visits to assess patient needs and provide clinical support.

Requirements

• Current LPN or RN license in South Carolina
• Minimum of two years of experience in maternal health, OB/GYN, community health, or chronic disease management
• Familiarity with managing patients with hypertension, diabetes, and high-risk pregnancy conditions
• Experience with electronic health records (EHR), preferably EPIC
• Ability to collect, analyze, and report patient data for program evaluation
• Strong communication and patient education skills
• Ability to work collaboratively with interdisciplinary teams
• Cultural competency and experience working with diverse populations

Nice To Haves

• Certification in Care Management (CCM) or Chronic Disease Management
• Experience with remote patient monitoring (RPM) technologies
• Prior experience working with community-based health programs or public health initiatives
• Bilingual (Spanish/English) preferred but not required
