About The Position

All Heart Home Care Agency is seeking a per diem Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) to support our NYC Human Resources Administration (HRA) home care program, with a focus on Emergency and Exceptional Home Attendant Services (also known as “Difficult to Serve” or DTS cases). This role provides psychosocial support and coordination for clients with complex medical, psychosocial, environmental, and family circumstances, and works closely with home attendants, agency staff, and outside case management entities to keep clients safely and successfully served in their own homes. This is a remote position. Candidates must hold an active, unrestricted New York State social work license (LMSW or LCSW) and registration, but do not need to reside in New York State — we welcome applicants located anywhere, provided they are able to meet the licensure and authorization requirements described below.

Requirements

  • Active, unrestricted New York State license and registration as an LMSW or LCSW (required).
  • Master's degree in Social Work (MSW) from a program accredited by the Council on Social Work Education.
  • Comfort and demonstrated competence conducting assessments and support sessions via telephone and video, including managing confidentiality, technology access barriers, and emergency situations remotely.
  • Reliable, private workspace with secure high-speed internet and a HIPAA-compliant device/setup for all client-related communication and documentation.
  • If residing outside New York State: candidate must independently confirm and attest, prior to starting, that providing telehealth/telepractice social work services to New York-based clients from their state of residence does not require a separate license or authorization in that state. Proof of NY licensure alone does not satisfy this requirement, and the agency will request a signed attestation as part of onboarding.
  • Ability to pass a criminal background check and provide employment eligibility documentation.
  • Willingness and ability to work with interpreter/language line services for clients with limited English proficiency.

Nice To Haves

  • Experience with home care, Medicaid-funded personal care services, aging/frail elderly, and/or physically disabled populations preferred.
  • Bilingual or multilingual candidates (Spanish, Russian, Haitian Creole) are strongly encouraged to apply.

Responsibilities

  • Provide psychosocial assessment and ongoing support for DTS/“difficult to serve” clients whose needs involve psychosocial, environmental, and/or family complexity.
  • Provide ongoing training, guidance, and intensive support to home attendants and housekeepers assigned to complex cases, to help them sustain consistent, quality care.
  • Coordinate with and make timely referrals to the appropriate case management entities (e.g., managed care plans, CASA offices, community-based services) to stabilize clients and connect them with needed resources.
  • Support efforts to reduce home attendant turnover and case attrition on complex cases through responsive, ongoing engagement.
  • Participate in identifying and addressing risk factors, including safety concerns, changes in client condition, and potential abuse/neglect situations, escalating promptly per agency and DSS/HRA protocols.
  • Maintain accurate, timely case documentation consistent with agency, HRA, and NYS DOH standards.
  • Collaborate with the nursing staff and administrative team as part of the interdisciplinary care approach for assigned cases.
  • Support the agency’s Quality Assessment and Improvement Program by contributing to case reviews and service-delivery quality efforts as needed.
  • Maintain client confidentiality and comply with HIPAA, agency policy, and the NASW Code of Ethics in all interactions, including telepractice encounters.
