The Per Diem Primary Care Provider (PDPCP) is responsible for acting as a clinical leader and providing quality primary care services to Lyon-Martin patients. This role helps to grow the overall practice and promote team functioning. The PDPCP collaborates with a supervising physician according to California state law and upholds a commitment to addressing racism, ableism, fatphobia, and gender and sexual discrimination within the organization and clinical practice. Under the supervision of the Medical Director, the PDPCP fills coverage gaps for staff medical providers. Responsibilities include taking medical histories, performing physical examinations, ordering and interpreting tests, diagnosing and treating illnesses and injuries, and making referrals. The PDPCP provides consultation and education to patients using a trauma-informed, harm reduction framework, covering areas such as chronic disease management, pain management, substance use disorder treatment, gender-affirming care, and gynecological care. The role also addresses the medical needs of patients with co-occurring disorders and barriers to care within the Patient-Centered Health Home model and works collaboratively with Mental Health Providers and Specialists as part of the integrated healthcare team. Evening and weekend work may be required.
