About The Position

This per-diem pharmacist position involves performing Medication Therapy Management (MTM) through in-person and telephonic consultations with patients and prescribers. The role focuses on promoting high-quality, evidence-based, and cost-effective pharmaceutical care. Responsibilities include comprehensive medication reviews, patient assessment and counseling, identifying potential drug interactions, and assessing patients for medication adherence barriers. The pharmacist will develop care plans, provide therapeutic recommendations to providers, educate patients on medication use, and document clinical consultations. The position also requires staying updated on chronic disease states and pharmacy practice changes, participating in continuing education, contributing to staff meetings, identifying areas for operational improvement, and adhering to all departmental and organizational policies and regulations. Assistance in the outpatient pharmacy may also be required.

Requirements

  • Doctor of Pharmacy degree
  • 1-3 years’ experience
  • Massachusetts License
  • Training or equivalent experience in Medication Therapy Management (MTM)
  • CPR Certification
  • Detail oriented
  • Ability to interact with diplomacy and tact
  • Ability to work with people from a variety of culturally diverse backgrounds
  • Reliable and professional in behavior and appearance

Responsibilities

  • Performing MTM in-person/telephonic pharmacotherapy consultations to patients and/or prescribers promoting high quality, evidence-based, cost-effective pharmaceutical care.
  • Completing a comprehensive medication review of prescription and non-prescription medications with emphasis on patient assessment and counseling, safe and effective use of medications, identification of potential drug interactions, and other related activities.
  • Completing pharmacotherapy assessment of patient’s claims, past medical history, and disease states.
  • Maintaining a high level of clinical knowledge of chronic disease states and changes in pharmacy practice in adult and geriatric patient populations to serve as an authoritative information source on drugs and their utilization in therapy.
  • Assessing patients for medication adherence barriers and develops a plan to maximize patients’ medication success through improving or maintaining adherence to medications.
  • Developing a plan of care that details actions of the patient and/or prescriber based on the Pharmacotherapy assessment.
  • Delivering high quality evidence-based therapeutic recommendations to providers.
  • Educate patients on their medications and promote effective medication use.
  • Documentation of clinical consultations including the plan of care into internal database.
  • Participation in continuing education sessions provided by the department.
  • Effectively participates in and contributes to staff meetings and other staff development opportunities and interdepartmental work groups.
  • Identifies areas of improvement where systems modifications could result in operational advancements and efficiencies.
  • Adhere to all departmental and organizational policies and procedures as well as adherence to all the applicable laws and regulations.
  • Assist in outpatient pharmacy as needed
