Per Diem Nurse Practitioner
About The Position
Circadia Health is a growth-stage healthcare AI company focused on preventing hospitalizations and transforming senior care. The Circadia Intelligence Platform integrates contactless sensing, predictive models, and enterprise integrations to detect adverse events early. This role involves working across skilled nursing and Home Health AI Care programs, providing direct patient care, acting on real-time health signals, and coaching facility teams to implement earlier interventions. The Nurse Practitioner will round on patients in SNF partner facilities and provide proactive care in patients' homes, blending direct patient care with technology integration through training and demonstrations. The position is based in Philadelphia, PA.
Requirements
- Certified and licensed Nurse Practitioner in PA.
- ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
- Eligible for Medicare participation.
- 2+ years NP experience with complex adult populations.
- Able to practice independently.
- Valid PA driver’s license.
- Comfortable traveling locally.
- Thrives in a dynamic, evolving startup environment.
- English required.
Nice To Haves
- Tech-savvy: Google Suite, multiple EMRs, Slack, AI transcription/tools.
- Spanish is preferred.
Responsibilities
- Conduct initial and follow-up assessments on flagged residents.
- Initiate/adjust plans of care.
- Complete consent forms.
- Coordinate with facility providers.
- Train and coach staff at the point of care, standardizing the use of Circadia into clinical workflows.
- Interpret trend-based insights from data.
- Prioritize high-risk patients.
- Drive timely interventions to prevent patient decline.
- Complete documentation/charts within the same day.
- Efficiently complete visits across designated SNFs (typically 25+/day where appropriate), focusing on new admits, facility-flagged patients, and Circadia alerts.
- Surface field insights to improve workflows and tech integration in a clinical setting.
- Support NP onboarding.
- Share best practices.
- Partner with clinical ops/product teams.
- Cover SNF sites and patient homes within a 30–40 mile radius.
