About The Position
Circadia Health is a growth-stage healthcare AI company focused on preventing hospitalizations and transforming senior care. The Circadia Intelligence Platform integrates contactless sensing, predictive models, and enterprise integrations to detect adverse events early. This role involves working across skilled nursing and Home Health AI Care programs, monitoring patients, acting on predictive health signals, and coaching facility teams to implement earlier interventions. The Nurse Practitioner will conduct patient assessments, coordinate care, train staff on technology integration, interpret data insights, and drive timely interventions. The role requires rounding on new admits, flagged patients, and Circadia alerts in SNF partner facilities, as well as providing proactive care in patients' homes. The position blends direct patient care with technology integration through training, demos, and assessments. The role is located in Wenatchee, WA, Ellensburg, WA, and Toppenish, WA.
Requirements
- Certified and licensed Nurse Practitioner in WA.
- ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
- Eligible for Medicare participation.
- 2+ years NP experience with complex adult populations.
- Able to practice independently.
- Valid WA driver's license.
- Comfortable traveling locally.
- Thrives in a dynamic, evolving startup environment.
- English required.
Nice To Haves
- Tech-savvy: Google Suite, multiple EMRs, Slack, AI transcription/tools.
- Spanish is preferred.
Responsibilities
- Conduct initial and follow-up assessments on flagged residents.
- Initiate/adjust plans of care.
- Complete consent forms.
- Coordinate with facility providers.
- Train and coach staff at the point of care, standardizing the use of Circadia into clinical workflows.
- Interpret trend-based insights.
- Prioritize high-risk patients.
- Drive timely interventions to prevent decline.
- Complete documentation/charts within the same day.
- Efficiently complete visits across designated SNFs (typically 25+/day where appropriate), focusing on new admits, facility-flagged patients, and Circadia alerts.
- Surface field insights to improve workflows and tech integration in a clinical setting.
- Support NP onboarding, share best practices, and partner with clinical ops/product teams.
- Cover SNF sites and patient homes within a 30–40 mile radius.
- Mileage and lodging are covered for locations outside the specified radius per company policy.
