About The Position

The Per Diem Nurse Logistic is responsible for collecting and interpreting clinical data from various sources to develop patient transfer plans. This includes assessing transfer eligibility, determining the appropriate level of care, prioritizing resources, ensuring timely acceptance, and selecting the appropriate priority and mode of transport. The role also involves acting as a clinical resource for all parties involved in patient transfers, communicating essential clinical and non-clinical information, and coordinating with transport providers to ensure safe and efficient transfers. A positive, proactive, problem-solving, and collaborative attitude is essential for success in this role.

Responsibilities

  • Collects and interprets clinical data from physicians, nurses, and charts for transfer planning.
  • Determines transfer eligibility, level of care, resource prioritization, and utilization.
  • Ensures timely acceptance and appropriate priority and mode of transport for patients.
  • Acts as a clinical resource to customers, departments, and clinicians involved in patient transfers.
  • Communicates clinical and non-clinical information to relevant parties, including nurses, physicians, and specialists.
  • Determines the type of physician required for transfers and contacts them to facilitate timely transfers.
  • Assists physicians/providers in determining the appropriate level of transport.
  • Coordinates with transport providers to ensure appropriate transport priority and mode.
  • Considers appropriate resource utilization and timeliness of transfer.
  • Works to achieve positive, timely, and safe patient transfers.
  • Practices with a positive, pro-active, can-do, problem-solving, and collaborative attitude.
