About The Position

Andrea's Angels is seeking multiple Per Diem Fee-for-Service Registered Nurses (RNs) to join their clinical team. This field-based position involves providing skilled nursing visits and conducting in-home patient assessments in Greeley and Northglenn, Colorado. The role is ideal for experienced, compassionate nurses seeking flexibility, autonomy, and the chance to make a significant impact in home healthcare. Compensation is per-visit, and flexible scheduling is offered.

Requirements

  • Active Colorado Registered Nurse (RN) license in good standing is required.
  • Minimum one (1) year of nursing experience preferred; home care experience is a plus.
  • Strong clinical assessment, critical thinking, and documentation skills.
  • Ability to work independently and effectively manage a field-based schedule.
  • Willingness to travel throughout Colorado.
  • Excellent interpersonal, communication, and organizational skills.

  • Home care experience is a plus.

Responsibilities

  • Provide skilled nursing visits in accordance with the patient's plan of care.
  • Conduct comprehensive in-home patient assessments, including Start of Care (SOC) and other clinical assessments as assigned.
  • Accurately evaluate patients' medical conditions, functional status, and home care needs.
  • Complete all required clinical documentation in a timely and accurate manner.
  • Develop and update patient plans of care in accordance with agency policies and regulatory requirements.
  • Collaborate with physicians, clinical leadership, Home Health Aides (HHAs), Personal Care Aides (PCAs), and the interdisciplinary care team to coordinate patient services.
  • Educate patients and caregivers on disease management, medications, safety, and home care services.
  • Report significant changes in patient condition to the appropriate clinical personnel.
  • Ensure compliance with all agency, state, federal, and Department of Health regulations.
  • Deliver compassionate, patient-centered care while maintaining professionalism during every patient interaction.
