This role assesses patients and develops and initiates treatment plans based on evaluation results. It involves reviewing physician referrals, patient conditions, and medical records to determine the necessary physical therapy treatment. The position provides direct patient care according to established protocols and guidelines, documents findings, progress, and instructions, and is responsible for the care performed by assistants and technicians. The therapist will create a written plan of care, develop goals based on evaluation findings, and implement a comprehensive program to achieve those goals. Effective communication with patients, caregivers, professional colleagues, physicians, and insurance companies is essential. Timely documentation, including daily notes, weekly reports, progress reports, initial evaluations, discharge summaries, and incident reports, is required.
