Transitions to Community Living (TCL) Peer Support Professional (3806) is a full-time position in Charlotte, NC. This role provides Peer Support Services to individuals with Mental Health and/or substance use disorders who are participating in the Transition to Community Living program and have Permanent Supportive Housing. The primary goal is to increase an individual’s ability to live successfully in the community by maintaining tenancy. This service is appropriate for individuals who are currently in stable housing but have continuing needs around maintaining tenancy, community inclusion, building supports, and enhancing quality of life. The Peer Support Services TCL collaborates with the client’s therapists to determine the individual’s status and response to treatment, performs symptom assessment and management, provides individual supportive counseling using the WRAP model, and assists with activities-of-daily-living, social, interpersonal relationship, and leisure-time-activity services. Services are highly individualized and provided in the community, promoting individual self-determination and decision-making. The role also involves collaborating with therapists and medical providers to ensure client preferences are integrated into treatment and rehabilitation. This position requires a non-traditional schedule, with availability as needed to meet clients at convenient times, including after-hours and crisis support, with 24/7 availability in the event of a crisis.
