About The Position

Community Vision is looking for a Direct Support Professional to join our Community Living team. The Community Living Specialist works one-on-one with multiple individuals receiving our services, providing direct support and facilitating various activities to ensure the individuals receiving our services have opportunities at home and in the community that create an optimum range of choice, respect, competency, community presence and relationships.

Requirements

Highly organized, adaptable, and has the ability to quickly establish healthy connections with the individuals receiving our services.
Frequent, daily travel is required. Must be available/able to drive to various locations in the Portland Metro Area and be willing to transport Supported Individuals in your own vehicle.
Must possess current liability insurance and have a valid Driver’s License.
Be able to push, pull, and lift up to 50 lbs.
Have at least one-year paid experience in developmental disabilities field, or in human services.
Obtain Approved Criminal History Check through the Oregon Department of Human Services Background Check Unit.
Not be on the OIG List of Excluded Individuals and Entities.

Responsibilities

Works with individuals and their support team in identifying and monitoring specific activities to achieve goals in their Individual Support Plan, according to the Service Agreement.
Provide individualized direct support, which may include financial management, medical management, grocery shopping, community connections, household maintenance, and creating and maintaining relationships through social skill development.
