About the Role: The Peer Support Specialist is an integral member of Community Reach Center’s outpatient residential, and community-based programming. The Peer Support Specialist assists consumers in their recovery by sharing their knowledge of the behavioral health system. Assists consumers articulating personal goals for recovery through one-to-one meetings and group sessions.

Requirements

  • Minimum requirement High School Degree or equivalent; Bachelor’s Degree preferred, but not required
  • Certified peer specialist preferred, but not required
  • Experience working with adults who have mental illness preferred, but not required
  • Communication, organization, and time management skills
  • Ability to work independently in a community-based setting when needed
  • Valid Colorado Driver’s license with a clean driving record
  • Bilingual Spanish preferred, but not required

Nice To Haves

  • Bachelor’s Degree preferred, but not required
  • Certified peer specialist preferred, but not required
  • Experience working with adults who have mental illness preferred, but not required

Responsibilities

  • Orients new consumer to the program and available services
  • Provide individual and group peer support to current ACT and ASCENT consumers
  • Meets with consumer to create goal – plans and personal safety plans when needed
  • Partners with integrated treatment team to develop and maintain a database of community resources available to consumers
  • Utilizes resource database by recommending best services for consumers to aid in their recovery needs
  • Maintains accurate clinical documentation according Community Reach Center policies.
  • If clinically appropriate transport and accompany consumers to appointments
  • Outreach and engagement of consumers
  • Provide recovery focused skill building
  • Engage in supporting and educating family of consumers
  • Collaborates with treatment team to provide additional treatment support
  • Willingness to be trained in and support the hospital navigation and hospitalization follow up process
  • Other duties related to position as assigned
