About The Position

This role supports and promotes a service environment for individuals in need of primary care, behavioral health, and/or co-occurring services. The environment should be recovery-focused, welcoming, and attentive to the needs of individuals who may have experienced trauma. The specialist will attend annual core competencies training related to individuals with primary care needs or those who have experienced health difficulties. They will establish and maintain professional working relationships with supervisors and peers, and work cohesively within a team or group setting. The role involves teaching and mentoring the value of each individual's recovery experience, modeling effective coping techniques and self-help strategies, assisting service recipients in articulating personal recovery goals, and helping them determine the objectives needed to reach those goals. The specialist will establish rapport with clients, assess their needs, organize, plan, and deliver face-to-face client services, and conduct follow-up on service outcomes. They will provide intensive services to assist clients in maintaining outpatient recovery, using individual assessments to determine service frequency. The role includes assisting clients in transitioning from higher to lower intensity services or to appropriate other programs, and advocating for effective recovery-oriented services. The specialist will assist with the referral process for individual needs, participate in outreach and engagement efforts for clients and professionals at inpatient levels of care to facilitate referrals and client participation, and assist consumers in obtaining services that suit their recovery needs. They will inform consumers about community and natural supports and how to utilize them in the recovery process. Maintaining strict confidentiality guidelines for consumer and family information is crucial. The role requires a minimum of 85 hours of billable services with consumers per month. Collaborative/Therapeutic Documentation is to be utilized to meet clinical record-keeping standards, ensuring documentation is completed with the consumer to the extent possible and on the same day of service. The specialist is responsible for seeking supervisory assistance for additional training and skill enhancement as needed. They will interact with other professionals to discuss therapy or treatment, new resources or techniques, and share information. The role serves as a liaison between inpatient psychiatric facilities to assist with discharge planning and follow-up care. Services are to be reported and documented in accordance with agency policy and procedures, with documentation completed by the end of the next business day. Evening or weekend hours may be required, as may providing services in a community and/or primary care setting. Client transportation using a personal vehicle or agency-owned van or small bus may be required. Other related duties may be assigned.

Requirements

High School diploma or GED
Demonstrated recovery from a mental illness, substance abuse disorder, or both.

Nice To Haves

Required to obtain certification for C-PRSS
Required to obtain training in health initiatives and annual updates.
Required to complete, Therapeutic Options, or approved safety training and annual updates.
May be required to obtain and maintain First Aid and CPR Certifications.

Provides and advocates for effective recovery-oriented services.
Assists with the referral process for each individual’s needs.
Participates in outreach and engagement efforts for clients and other professionals at inpatient levels of care to facilitate referrals and client participation in services.
Assists consumers in obtaining services that suit that individual's recovery needs.
Informs consumers about community and natural supports and how to utilize these in the recovery process.
Responsible for maintaining the strictest confidentiality guidelines for interactions regarding consumer and family information.
Provides a minimum of 85 hours of billable services with consumers per month.
Utilizes the philosophy of Collaborative/Therapeutic Documentation to meet clinical record-keeping documentation standards and ensure documentation is completed with the Consumer to the extent possible and all documentation is completed on the same day of the date of service.
Responsible for seeking out supervisory assistance for additional training and to enhance skill set as needed.
Interacts with other professionals to discuss therapy or treatment, new resources or techniques, and to share information.
Works as a liaison between inpatient psychiatric facilities to assist in discharge planning and follow-up care from inpatient treatment.
Reports and documents services in accordance with agency policy and procedures.
Adheres to HOPE policy and procedure of having documentation completed by the end of the next business day.
May be required to work evening or weekend hours.
May be required to provide services in a community and/or primary care setting.
May be required to provide client transportation using personal vehicle or agency owned van or small bus.
Performs other related duties as assigned.
