•$22 - $33•Onsite

About The Position

Healthcare in Action provides street medicine to people experiencing homelessness, using medically equipped vans to deliver care in settings such as parking lots, encampments, and parks. The Peer Navigator supports members who are experiencing or at risk of homelessness in Hollywood, West Hollywood, and Koreatown, California, helping them manage their health through education, peer support, housing and resource navigation, and shared experiences that can encourage positive behavior changes. The role supports Enhanced Care Management (ECM), CalAIM Community Supports (CSS), and 30-day hospital discharge response and care in partnership with Cedars-Sinai Medical Center. ECM uses a whole-person approach to coordinate clinical and non-clinical needs for high-need Medi-Cal beneficiaries. CSS includes housing navigation, tenancy-sustaining services, and temporary or transitional housing supports through Medi-Cal. This is a patient-facing, full-time position involving an eight-hour weekday shift in the field, scheduled between 8 a.m. and midnight. Flexibility is required because hours may be early or extend into the evening. The position may occasionally involve driving a medically equipped van and requires travel of 50% or more. The Peer Navigator works under the supervision of a Social Worker and supports Healthcare in Action’s goals, particularly housing placement for unsheltered members. Employees may be eligible for differential pay.
