Peer Driver
About The Position
Provide transportation and peer support to treatment services for clients with opioid use and substance use disorder. This role will require driving to various community-based stakeholders across Long Island and other boroughs. Field-based and on-site peer support to prospective clients. Assist client with person-centered recovery goal planning. Collaborate with client and care management team to identify the services clients should receive based on stage of change, immediate needs, and recommended treatment objectives. Provide services to clients to address housing, income support, education and vocational training, social supports, employment, and primary care needs. Assist clients with linkages to and systems navigation within the behavioral health, human services and criminal justice systems as appropriate. Act as an advocate and liaison for clients in accessing resources to support treatment plan goals. Use motivational interviewing, recovery and trauma informed approaches when delivering peer services. Involve family and significant others in the client’s treatment with the team. Provide individual peer support, wellness counseling, harm reduction counseling based on motivational interviewing. Accompanying clients to appointments and events. Conduct outreach to clients who unexpectedly disengage from clinic services.
Requirements
- Valid driver's license
- Experience with motivational interviewing, recovery, and trauma-informed approaches
- Ability to collaborate with multi-disciplinary teams
- Ability to assist clients with person-centered recovery goal planning
- Ability to provide services addressing housing, income support, education, vocational training, social supports, employment, and primary care needs
- Ability to assist clients with system navigation
- Ability to act as an advocate and liaison for clients
- Ability to provide individual peer support, wellness counseling, and harm reduction counseling
- Ability to accompany clients to appointments and events
- Ability to conduct outreach to disengaged clients
