About The Position

Join an innovative care model that combines the clinical excellence of outpatient rehabilitation with the convenience and comfort of speech therapy services delivered where patients live. This position is part of a new at home program serving outpatient therapy across north Charlotte, NC. This therapist will provide speech-language pathology services for pediatric patients in the patient's home in Huntersville and North Charlotte areas. Caseload will include birth-18 years of age. Preference for a background in pediatric feeding and swallowing disorders. This position is not eligible for a clinical fellowship. Ideal for clinicians who value autonomy, relationship-based care, clinical excellence, innovation, and expanding access to high-quality speech-language pathology services within the community. This therapist will provide speech-language pathology services for pediatric patients in the patient's home in Harrisburg/ Concord areas. Caseload will include birth-18 years of age. Preference for a background in pediatric feeding and swallowing disorders. Deliver high-quality, one-on-one outpatient speech-language pathology services in patients’ homes, helping improve communication, cognition, swallowing function, and overall quality of life. Provide the same evidence-based outpatient speech therapy services traditionally offered in a clinic while eliminating barriers related to transportation, access, and convenience. Perform comprehensive speech-language and cognitive-communication evaluations, develop individualized treatment plans, and implement interventions tailored to each patient’s unique goals and functional needs. Collaborate closely with physicians, caregivers, and interdisciplinary team members to support optimal outcomes in communication, cognitive function, swallowing safety, and patient independence. Utilize technology and flexible scheduling to effectively manage a caseload across a defined geographic area while maintaining exceptional patient outcomes, engagement, and satisfaction.

Requirements

  • Master's Degree in Speech Language Pathology required.
  • Applicable state license for Speech Language Pathology required.
  • BLS required per policy guidelines.

  • Background in pediatric feeding and swallowing disorders preferred.

Responsibilities

  • Provides patient evaluation and care planning for speech-language pathology.
  • Delivers patient care appropriate to age specific and other population needs.
  • Provides clinical leadership.
  • Assesses and documents patient's medical, mental and emotional needs at admission and on an on-going basis.
  • Develops and implements a plan of care to meet patient and family needs, to include discharge planning and utilization of available resources.
  • Acts as patient advocate; treats patients and families with compassion and implements plan of care in a safe and timely manner.
  • Demonstrates clinical reasoning, coupled with clinical skills to conduct accurate clinical assessments and perform patient care activities; evaluates effectiveness of therapy interventions, identifies and prioritizes patient/family strengths, needs and priorities.
  • Identifies the normal course of illness in assigned patient population recognizing and intervening appropriately when deviations occur.
  • Communicates patient information and thoroughly documents therapist's actions and plan of care.
  • Provides clinical leadership and mentoring.
