About The Position

Imagine Pediatrics is a tech-enabled, pediatrician-led medical group dedicated to reimagining care for children with special healthcare needs. We provide 24/7 virtual-first and in-home medical, behavioral, and social care, collaborating with families, providers, and health plans to overcome barriers to quality care. Our role is to enhance existing care teams, offering an additional layer of support with compassion, creativity, and a steadfast commitment to children with medical complexity. The primary location for this position is remote, requiring residency in the Eastern Time Zone, with quarterly travel to Tampa, FL. The expected schedule is 3x12-hour shifts (Monday-Wednesday or Wednesday-Friday) from 7:00 am to 7:00 pm EST. Nurses must be physically located within the United States.

Requirements

  • Licensed RN in at least one state with eligibility to register for other state licensures.
  • Bachelors in nursing from an accredited university required.
  • 5+ years of pediatrics experience required.

Nice To Haves

  • Outpatient (primary care and/or subspecialty), home health, complex care, pediatric ICU, emergency medicine, etc. strongly preferred
  • 1+ years' experience in care coordination or case management experience preferred
  • Bilingual Spanish strongly preferred
  • Familiarity with Medicaid regulations and services a plus
  • Value Based Care (VBC) experience a plus
  • Virtual care experience a plus

Responsibilities

  • Provide professional and friendly proactive care and triage for clinical issues.
  • Embed a family-centered care philosophy in care delivery.
  • Demonstrate cultural competence and sensitivity, working with diverse populations and seeking additional resources when needed.
  • Manage care transitions for ED/IP/UC, coordinating with clinical providers post-discharge.
  • Perform comprehensive assessments of a patient’s clinical, psychosocial, discharge planning, and financial needs, establishing clinical milestones and goals.
  • Establish rapport and relationships with patients and families to understand their needs, expectations, and assist in setting realistic, mutual goals, integrating cultural awareness into the interview process.
  • Establish a comprehensive plan of care, in conjunction with the physician and interdisciplinary team, to address clinical milestones.
  • Communicate the plan of care, including changes and related issues, to patients/families, physicians, and other healthcare team members.
  • Gather sufficient information from all relevant sources to determine the effectiveness of the plan of care, ensuring it is accurate, safe, timely, and cost-effective.
  • Document all care management assessments and interventions.
  • Refer to Social Work or Behavioral Health for complex psychosocial and discharge planning issues (per criteria) and ensure appropriate follow-up.
  • Consult with other members of the interdisciplinary team (dietary, pharmacy, etc.) to ensure safe discharge as appropriate.
  • Perform other duties as assigned.
