About The Position
Affirmed Home Care is seeking compassionate and experienced Registered Nurses (RNs) to support multiple pediatric home care cases throughout Wayne, Pompton Plains, and Franklin Lakes, NJ. These cases offer a mix of weekend, overnight, and evening opportunities, making them ideal for nurses seeking part-time, full-time, or supplemental hours. The available cases include a 15-year-old trach and ventilator dependent patient in Wayne requiring weekend 16-hour coverage, a pediatric patient in Pompton Plains requiring trach, ventilator, and G-tube care with overnight skilled nursing support (11:00 PM – 7:00 AM shifts, approximately 40 hours per week), and a pediatric patient in Franklin Lakes requiring private duty nursing support with transportation and care coordination assistance, offering Monday, Tuesday & Wednesday evening coverage and Sunday 1:00 PM – 7:00 PM.
Requirements
- Active New Jersey RN License
- Current BLS Certification
- Authorized to work in the United States
- Physical exam within 12 months
- PPD or QuantiFERON within 1 year (or chest X-ray within 5 years)
- Proof of flu vaccine or approved exemption
Nice To Haves
- Experience with trach, ventilator, and G-tube care preferred
- Valid driver's license and reliable transportation preferred
Responsibilities
- Provide 1:1 pediatric nursing care in the home setting
- Monitor and manage tracheostomy and ventilator support as ordered
- Administer and monitor G-tube feedings
- Perform comprehensive patient assessments and skilled nursing interventions
- Administer medications and treatments per physician orders
- Develop and implement individualized plans of care
- Maintain accurate clinical documentation
- Collaborate with families, physicians, and the pediatric clinical team
