About The Position

This case involves a resilient 1-year-old girl with a history of Trisomy 21 and several cardiac repairs (AVSD/PDA). She is currently preparing for discharge from the hospital, and we are building her "dream team" to ensure she has a safe and happy return home. The patient is a 1-year-old female requiring Trach/Vent management and monitoring for feeding intolerance. She has a medical history of respiratory failure and cardiac repairs. The home environment is very welcoming with parents who are highly involved and helpful with her care. The household is home to 5 cats and 2 lizards (caged), so the ideal nurse must be very comfortable with pets! Requirements

Active RN or LPN license in the state of Michigan
Experience in home health, private duty, or long-term care preferred
Pediatric or adult care experience a plus (trach/vent knowledge helpful but not required)
Strong communication skills and a passion for patient-centered care

Responsibilities

Provide skilled nursing care (trach/vent care, medication administration, catheter care, etc.)
Monitor and document vitals, symptoms, and changes in condition
Assist with ADLs such as repositioning, hygiene, mobility, etc.
Support patient well-being with compassionate and attentive care
Collaborate with families, caregivers, and the healthcare team
