About The Position

Trillium Family Services is seeking a compassionate and experienced Primary Care Provider to deliver on-site medical care for youth residing in our residential treatment programs. This role focuses on completing admission history and physical examinations, providing routine primary care services, and collaborating with interdisciplinary teams to support the overall health and well-being of children and adolescents receiving behavioral health treatment.

Requirements

  • Licensed Physician (MD, DO or NP)
  • Current DEA registration, if applicable to licensure and scope of practice
  • Malpractice insurance
  • Knowledge of common pediatric and adolescent medical conditions, preventive health practices, and chronic disease management
  • Ability to work effectively within an interdisciplinary behavioral health treatment environment
  • Strong communication and collaboration skills
  • Experience with electronic medical records and clinical documentation systems

Nice To Haves

  • Experience providing primary care to pediatric or adolescent populations preferred
  • Experience working with children and adolescents in residential treatment, behavioral health, foster care, school-based health, or community health settings
  • Familiarity with trauma-informed and culturally responsive care practices
  • Understanding of the medical needs commonly associated with youth receiving behavioral health services

Responsibilities

  • Complete comprehensive history and physical (H&P) examinations for youth upon admission to residential treatment programs
  • Conduct routine primary care visits for acute illnesses and minor medical concerns
  • Evaluate and manage common pediatric health conditions within the scope of primary care practice
  • Review medical histories, medications, immunization records, and relevant health documentation
  • Order and interpret routine laboratory tests and diagnostic studies as clinically indicated
  • Provide basic health education and preventive care recommendations
  • Collaborate closely with psychiatric providers, nursing staff, therapists, and residential treatment teams to support integrated care
  • Document all patient encounters accurately and in a timely manner within the electronic health record
  • Coordinate referrals to specialty care providers when appropriate
  • Assist in developing and maintaining standards of medical care within the residential treatment setting
  • Support compliance with agency, regulatory, licensing, and accreditation requirements
