About The Position
Join a physician-led primary care organization built around outcomes—not volume. This role offers manageable patient panels, strong care team support, and the ability to practice medicine the way it was intended: with time, accountability, and purpose. This is a value-based, full-risk Medicare Advantage model with a patient panel capped at approximately 450 patients. The focus is on prevention, care coordination, and behavior change, with dedicated care teams (MA, Care Coordinator, Care Facilitator). There is an emphasis on quality, patient experience, and physician wellness.
Requirements
- MD or DO required
- Board Eligible or Board Certified in Internal Medicine, Family Medicine, or Geriatrics
- Active or eligible state medical license
- Active DEA
- Commitment to value-based, team-oriented care
Responsibilities
- Provide comprehensive primary care to older adult patients
- Maintain full accountability for clinical outcomes and patient experience
- Serve as a health coach, guiding lifestyle and long-term health improvement
- Coordinate specialty and hospital care while staying actively involved
- Lead and collaborate with an interdisciplinary care team
- Supervise and remain accountable for Advanced Practice Providers (APPs)
- Partner with medical and operational leadership to meet quality and cost goals
