About The Position

Angels of Care Pediatric Home Health is seeking Physical Therapists (PTs) across Colorado to provide in-home therapy services for children with developmental, physical, and neurological needs. This role is ideal for clinicians passionate about helping patients build independence and improve quality of life in a supportive, home-based setting. Requirements

Active, unrestricted Physical Therapist license (Colorado)
Graduate of an accredited Physical Therapy program
Current CPR certification
Strong communication and interpersonal skills
Must submit a resume to be considered for this position

Nice To Haves

1+ year of pediatric experience (new grads welcome!)
Home health experience

Responsibilities

Provide evidence-based pediatric physical therapy in the home and community
Perform evaluations, assessments, and ongoing progress monitoring
Develop and implement individualized treatment plans and goals
Deliver interventions to improve mobility, strength, balance, coordination, and motor development
Educate and empower families and caregivers for carryover in daily routines
Collaborate with physicians, caregivers, and interdisciplinary teams
Document care in an electronic medical record system
Maintain compliance with all clinical and regulatory standards
