The VNA & Hospice of the Southwest Region is looking for a per diem Pediatric Physical Therapist in Bennington County. Requirements
- Bachelor's degree in Physical Therapy from an accredited university or college required.
- Vermont State Physical Therapist license required.
- Valid State Driver's license required.
- Ability to successfully pass pre-employment driver record, criminal, adult and child abuse background check/review required.

- Master's degree preferred.
Responsibilities
- Under the direction of the Kids on the Move Clinical Manager, evaluate, plan, and administer medically prescribed or educationally relevant physical therapy treatment to clients in their homes or students in their respective educational environment.
- Provide clinical supervision of students, physical therapy assistants and rehabilitation aides.
- Actively monitor and amend patient visit schedule to maximize productivity and patient need, accounting for new patients and changes in existing patient schedule.
- Perform a comprehensive PT assessment of assigned patients including, but not limited to psychosocial, educational, safety and risk factor parameters.
- Develop a plan of care to promote optimal functional independence and safety.
- Plan for and provide skilled physical therapy treatment including, but not limited to, therapeutic techniques, patient/caregiver education, assessing the patient's status and progress on each visit and ensuring quality of care.
- As Care Manager and/or team member, coordinate patient care with other clinical team members, including physicians, nurses, caregivers, other rehab services and students, ensuring optimal patient care and communication.
- Accurately and completely perform functions as related to school-based services, including evaluations, documentation, development of Individual Educational Plan (IEPs), participation in meetings, and provision of direct and consultative services.
- Fill out and complete insurance paperwork in a timely manner.
- Coordinate safe patient discharges and/or transfers and ensure all appropriate paperwork and patient information is processed.
- Serve as a Home Care PT as required.
