About The Position

Eisner Health is seeking a Pediatric Nurse Practitioner to provide services to underserved communities. The successful candidate will be responsible for direct patient care, multidisciplinary interaction with support services, and other clinical and administrative responsibilities. Utilizing guidelines from the American Academy of Pediatrics and the Department of Health’s Child Health and Disability Prevention Program (CHDP), the nurse practitioner will provide primary care to pediatric patients aged 0-18.

Requirements

  • License to practice as a nurse practitioner in the State of California.
  • Board certified or board eligible in the field of Pediatrics.
  • At least 2 years of clinical experience as a nurse practitioner in the area of specialty.
  • Effective communication and interpersonal skills with children and families.
  • Maintain CPR certification.

Nice To Haves

  • Knowledge and experience in managed care preferred.
  • Proficiency in Spanish is highly preferred.

Responsibilities

  • Serve as the Primary Care Provider (PCP) for assigned patients, conducting initial evaluations and subsequent visits according to HMO guidelines to address immediate needs.
  • Perform medical evaluations, including history and physical examinations, and order laboratory tests, X-rays, referrals, and other diagnostic studies.
  • Evaluate patients, develop problem lists, and establish treatment objectives and plans.
  • Initiate treatment orders and procedures.
  • Collaborate with other staff members to coordinate and obtain necessary services for patients.
  • Educate and counsel patients and their parents on health promotion and disease prevention.
  • Use and disclose protected health information in compliance with HIPAA standards.
  • Practice within the Center’s established treatment protocols and under the direction of the supervising physician.
  • Consult with the physician supervisor on any diagnostic evaluation or medical treatment questions.
  • Follow through personally with initially evaluated patients, scheduling return visits as necessary.
  • Record visit forms in the EHR system, electronically signed, detailing changes in patient condition, diagnostic studies, active problems, diagnoses, treatments, and the logic of evaluations and plans.
  • Document pertinent patient or family attitudes/behaviors towards patient care.
  • Ensure continuity of care by other providers through clear documentation.
  • Provide specialty clinic services if medically necessary and integrate recommendations from subspecialists into the patient treatment plan.
  • Inform patients about their medical problem, prognosis, and the nature and purpose of contemplated treatments and procedures.
  • Provide and document appropriate self-care instructions to patients.
  • Generate a second opinion referral if requested by the patient’s family and deemed medically reasonable.
  • Generate and maintain a current chronic problem list.
  • Address items in the PAQ on a regular basis.
  • Promote a cohesive, cooperative working relationship with fellow providers, including assisting with excess patient loads and staying after scheduled hours.
  • Maintain excellent professionalism when dealing with patients.
